Service Description

We provide consultations to help you navigate the healthcare community. We identify problems, develop workable solutions, and provide coaching to clients and/or families by evaluating, planning, and coordinating your care with you. Includes hospital discharge planning, transitioning to another level of care, working with insurance, and basic healthcare navigation. Service can be provided via phone, face-to-face, or virtual.
